Value-driven health care models that aim to improve patient outcomes while managing total costs of care require better access to integrated health care data. AMCP convened a forum in Baltimore, MD, on September 25-26, 2018. The goal of the forum was to develop a roadmap for managed care entities interested in managing medications across the spectrum of health care—regardless of whether the medications are covered by a medical or pharmacy benefit. During this webinar faculty will cover the proceedings from this forum, presenting a roadmap for managed care organizations interested in data integration.
