The healthcare landscape continues to evolve toward more coordinated, cost-effective, and team-based care in new payment and delivery models. ACOs, PCMHs and other innovative models will further shape the opportunities for pharmacists to deliver an increasing range of services to improve medication adherence and engage patients to improve overall health. We will provide current examples of innovative approaches to leveraging pharmacist services within these models and discuss our views on what the future may hold for developing payment and delivery models that integrate pharmacist services more directly in providing patient care.
