Format for Formulary Submissions Committee

Committee Mission:   

The mission of the AMCP Format Committee is to:  

• Guide AMCP in evaluating and updating the AMCP Format for Formulary Submissions (the AMCP Format) to ensure that it continues to evolve to meet the needs of health care decision makers who review clinical and scientific evidence and health care economic information when evaluating new drugs and technologies for formulary and coverage determinations. 
• Provide recommendations and input for the development of related training and educational materials for use in promoting understanding and use of the AMCP Format.   
• Guide AMCP in developing training programs, resources, and other enduring materials to improve understanding of the formulary decision-making process; assist Pharmacy and Therapeutics (P&T) committees and other health care decision makers evaluating clinical and scientific evidence and health care economic information for formulary and coverage decisions; and advance current practices in these areas. 

Committee Qualifications:   

• Committee members must be Active Members of AMCP and have experience with formulary decision making or developing submissions using the AMCP Format for Formulary Submissions.  

Term/Special Requirements:   

• Committee member terms are two years with eligibility to serve up to two terms. Terms are not automatically renewed.
