Patient Centered Medical Homes

The Patient Centered Medical Home (PCMH) represents an effective model for managing patient care by leveraging the primary care provider to help coordinate and deliver healthcare services. According to the National Committee for Quality Assurance (NCQA), “the patient-centered medical home is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into "what patients want it to be." Medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care.”

• Blue Cross Blue Shield Blue Care Network of Michigan The BC/BS/BCN of Michigan site includes an FAQ tailored to its members and interpretive guidelines for PCMH and PCMH Neighbor Domains of Function. (FAQ, Guideline, 2013)
