Patient Centered Medical Homes
The Patient Centered Medical Home (PCMH) represents an effective model for managing patient care by leveraging the primary care provider to help coordinate and deliver healthcare services. According to the National Committee for Quality Assurance (NCQA), “the patient-centered medical home is a way of organizing primary care that emphasizes care coordination and communication to transform primary care into "what patients want it to be." Medical homes can lead to higher quality and lower costs, and can improve patients’ and providers’ experience of care.”
The AMCP Special Projects Sub-Committee developed this resource guide to document and explore medication management practices of pharmacists in Patient Centered Medical Homes (PCMH). Click the below links to access this helpful information!
