CareOregon is a central part of an innovative virtual integrated delivery system focusing on population health, community well-being through providing a strong provider network and community support. CareOregon clinical pharmacy services are designed to assist with high risk medication management and utilize a communication system where all affected pharmacists can work together towards seamless patient care regardless of the patient’s care setting. Our PGY2 pharmacy residency in Ambulatory Care is designed to transition PGY1 residency graduates from generalist practice to specialized practice focused on the clinical ambulatory management and coordination of CareOregon patients throughout their continuum of care. Graduates of our Ambulatory Care PGY2 pharmacy residency will be well-equipped to provide exemplary patient-centered medication therapy and initiate and establish successful clinical pharmacy services in diverse healthcare settings. Our 12-month program will incorporate both outpatient and inpatient rotations and will follow the Ambulatory Care specific Specialty Residency Standards and Objectives established by ASHP. Our ASHP accreditation visitation is anticipated for the spring of 2016 during our inaugural residency year 2015-2016. We are confident that our ambulatory care residents will complete our program a highly competent clinician capable of establishing a clinical service and teaching program in any ambulatory care setting. In addition, the resident will be poised to contribute to the advancement of ambulatory pharmacy practice nationally.

Special Features: 
Our PGY-2 Ambulatory Care Residency is an innovative postgraduate program of clinical experience in outpatient settings. The resident will gain a broad range of experience within a Coordinated Care Organization (CCO); working closely with providers, support staff and community partners. Our resident will participate in the direct management of high-risk Medicaid and Medicaid patients. Interventions may include MTM, physical assessment, triaging, initiating and modifying medication therapy, ordering labs, and provision of patient and/or provider education. What sets this residency apart is its complete continuity of clinical care. In August after licensure is obtained each resident will design, build and implement ambulatory services within CareOregon primary care clinics throughout the residency. In addition, the curriculum allows for the pursuit of specific interests that best complement career goals. This is achieved by choosing from a variety of core and elective clinical rotations that are structured to build upon previous learning experiences.
